CSM Program is available to adults over 18 years of age who have been diagnosed with developmental and intellectual disabilities. Also, for individuals who are already living independently/transitioning into independent living and require life skill support to function successfully in the community.

Collaborative Skills Management aims to help all of our clients live the healthiest lives they can. In order to reach this goal, we offer a variety of services. We provide a health and fitness plan, and the positive reinforcement you need to make big lifestyle changes. Collaborative Skill Management SLS

Our mission at Collaborative Skill Management is to provide the necessary level of support for individuals who have made a decision to live in their own home.

 

Collaborative Skill Management shall encourage individuals with developmental and/or intellectual challenges in order to take control of and be responsible for their life choices.

 

Collaborative Skill Management shall provide an array of supportive services to our individuals. Collaborative Skill Management shall facilitate making informed choices concerning where and how they shall live. We shall ensure successful community living options by providing individualized support based on each individual’s needs.

Collaborative Coordinated Family Services

Our mission at Collaborative Coordinated Family Services shall facilitate making informed choices concerning the services

the individual and their family may be missing. Collaborative Coordinated Family Services shall fill the gap in the services for the individual, which may include providing assistance, support, and training for the individual and their family in navigating comprehensive services and supports that are tailored to meet their unique needs. Also, creating pathways to overcome barriers to accessing generic and other resources.

Collaborative ILS

Collaborative ILS’ curriculum provides extensive and modern methods to achieve the anticipated individual outcomes outlined above. The curriculum is designed to engage the individual using modern technological tools and instruments, cause individuals to retain knowledge for practical and real-life application and teach the individual a certain thought-pattern to make wise decisions similar to those of non- disabled adults.

 

Collaborative ILS aims to present instructional material using a variety of presentation methods to cover the different learning styles among individuals as some are visual, some auditory, etc. The various components of Collaborative ILS’ curriculum are detailed below.

Also, individuals will assist, to the best of their ability, in training onboarding ILS instructors to learn about them, their residence and their current IPP objectives. Individuals will provide input on how they want their services delivered, the frequency of those services and may also choose to deny said services.

Collaborative Parenting

Our mission at Collaborative Parenting is to enhance the well-being of families and children by providing education, counseling, and practical assistance to parents. Our goal is to provide parents with the resources they need to address their challenges and become stable and safe caretakers for their children.

Collaborative Parenting offers court approved parenting classes and issues certificates-Court acceptance guaranteed. Collaborative Parenting provides resources that help felicitate the reunification of children in the Department of Children and Family Services (DCFS) or Foster Care with biological families.
